GRANTS

For nearly 60 years, Kaiser Permanente’s social mission has been the foundation of its community service programs. As a values-driven, not-for-profit integrated health care organization, Kaiser Permanente is dedicated to improving the health of our members and the communities we serve.

Through partnerships with community organizations and governmental entities, Kaiser Permanente strives to benefit the community by addressing issues and concerns that affect overall community health. Many of these partnerships are created through grants that support community events, activities, projects and programs.

The Kaiser Permanente Santa Clara Market Area Grants Program responds to requests from organizations that provide service throughout Santa Clara Area communities: Santa Clara, Milpitas, Campbell, Mt. View, North San Jose, West San Jose, Cupertino, Saratoga, Los Gatos and Campbell.

Community Benefit Grants Program

Every three years, Kaiser Permanente undertakes a strategic planning process with the Community Benefit Coalition of Santa Clara County consisting of diverse public and private stakeholders.  It collects and analyzes important health data and trends to assess community health needs in order to develop a comprehensive plan to help improve the health of our communities. The Grants Program provides the opportunity to collaborate with locally focused organizations and agencies that provide direct services, or impact public policy related to improving the health and well being of our communities, based on the Medical Center’s local funding priorities.

Priority Health Goals 2008-2010

  1. To Improve and Promote Healthy Lifestyles for Youth.

    Programs need to aim at improving and promoting healthy lifestyles for youth, with an emphasis on obesity, healthy eating and active living programming that targets the family.  Proposals with the goals of serving the underserved or vulnerable youth building a positive a self image, providing health education and prevention programs to youth who are engaging high risk activities wil still be considered, however, those aiming to address and have a systemic impact in improving the overweight and obese youth in the county will be prioritized.

  2. To Increase Access to Health Care to the Underserved – Due to Income, Language, Culture or Age Barriers.

    Programs need to aim at improving access to health care services for the underserved populations with emphasis on older adults, persons with chronic diseases and families. Special emphasis to reach the underserved – those that are un-insured or under-insured, residents who speak English as a second language or experience other barriers to obtaining care.

Grant Guidelines

Before completing the Grants Application Form, please take a moment to read the guidelines presented below:

Kaiser Permanente grants are limited to organizations that are exempt from taxation under section 501 of the Internal Revenue Code.

Kaiser Permanente's Grants Program does not fund:

Religious (for religious purposes), partisan political, fraternal, athletic, international or social organizations, endowments, memorials or individuals, field trips or tours. Fundraising events such as raffles, telethons, walkathons and auctions are also ineligible.

Funding must be used for the program for which funds were requested.

Agencies who previously received Kaiser Permanente grant funds are eligible to reapply each year and will be considered along with all other qualifying proposals.

Funding decisions are made based on recommendations from the grants review committee and availability of resources.

Santa Clara Medical Center and Service Area Grants Application Forms

  1. Please submit the completed proposal and required supplemental attachments.
  2. Please see attached Grant Checklist for important deadlines
  3. Please be advised that one original and three copies of the proposal need to be submitted at the address below by noon of Thursday, March 27, 2008.  Also please email your proposal and budget document for expeditious confirmation and processing.
  4. Kaiser Permanente will contact you to notify you whether or not your organization’s proposal has been funded by June13, 2008.

To be eligible for a grant, you will need to print out the proposal, line item budget and check list, fill it out completely and mail it to the address provided below. Adobe Acrobat Reader is required to view the Request for Proposal Announcement.
